Cystic Fibrosis Benefit Buddy Bass Tournament - Entry Form

In submitting this application, we hereby waive and release all other contestants, sponsors, and tournament officials from all claims for injury and/or damages incurred in connection with this tournament. QUALITY ASSURANCE TEST: By his/her submission of this entry form and/or participation in this tournament, contestants agree to submit to and successfully pass the quality assurance tests. These may include all or part of the following: Polygraph tests, urinalysis, blood tests, and observer, in or out of boat. Refusal to submit by, either or both anglers, will result in immediate disqualification.
